Key Requirements for Pag-IBIG Multi-Purpose Loan Application
Multi-Purpose Loan Application Form (MPLAF, HQP-SLF-065)
Applicants must submit one original copy of the completed MPLAF. This form serves as the official application document and must be filled out accurately to avoid delays in processing.
Valid Identification (ID)
A photocopy of a valid ID that is recognized by the Pag-IBIG Fund must be included with the application. The original ID should also be presented for authentication purposes.
Proof of Income
Depending on the applicant's employment status, different proof of income is required.
For Formally-Employed Individuals:
The “Certificate of Net Pay” portion on the back of the application form must be completed by the employer.
Alternatively, a photocopy of the most recent payslip, authenticated by the company’s authorized signatory, may be submitted.
For Self-Employed Individuals:
Submit one of the following documents:
Income Tax Return (ITR), Audited Financial Statements, and Official Receipt of tax payment supported with DTI Registration and Mayor’s Permit/Business Permit (1 Photocopy).
Commission Vouchers reflecting the issuer’s name and contact details for the last 12 months (1 Original).
Bank Statements or Passbook for the last 12 months (for income from foreign remittance, pension, etc.) (1 Original).
Certified True Copy of Transport Franchise issued by the appropriate government agency (LGU for tricycles, LTRFB for other PUVs) (1 Original).
Certificate of Engagement issued by the business owner (1 Original).
Notarized Affidavit of Income (HQP-SLF-136) (1 Original).
For Overseas Filipino Workers (OFW):
Submit one of the following:
Employment Contract between employee and employer (1 Photocopy); or POEA Standard Contract (1 Original).
Certificate of Employment and Compensation (CEC) written on the employer’s official letterhead (1 Original); or signed by the employer (for household staff and similarly situated employees), supported by a photocopy of the employer’s ID and passport.
Income Tax Return filed with the host country/government (1 Original).
Payroll Account/Disbursement Card
For releasing loan proceeds, the applicant must submit a photocopy of their Payroll Account or Disbursement Card.
If applying through an employer, the photocopy must be certified by the employer.
For individual applications, the original Payroll Account/Disbursement Card should be presented for authentication.
Steps to Apply for a Pag-IBIG Multi-Purpose Loan
Complete the MPL Application Form (HQP-SLF-065):
Fill out the necessary details and ensure the employer completes the required sections if you're formally employed.
Gather All Required Documents:
Prepare photocopies of the valid ID, proof of income, and Payroll Account/Disbursement Card.
Submit the Application:
You can submit the application through your employer or individually at any Pag-IBIG branch.
Wait for Approval and Release of Loan Proceeds:
Once your application is approved, the loan proceeds will be disbursed through your Payroll Account or Disbursement Card.
